Diabetic peripheral neuropathy (DPN) is the most common chronic complication of diabetes, affecting roughly half of people with the disease over their lifetime. It is also the single most important risk factor behind diabetes-related foot ulcers and lower-limb amputations, because the loss of protective sensation allows minor injuries to go unnoticed until they become serious wounds. Yet DPN often develops silently, and a substantial proportion of cases remain undiagnosed until complications appear. Understanding how the condition is detected and managed is therefore central to protecting the diabetic foot.

Why Early Detection Matters

Nerve damage in diabetes typically begins in the longest nerves first, producing a symmetric “stocking-and-glove” pattern that starts in the toes and feet. Some people experience burning, tingling, shooting pain, or numbness, while others feel nothing at all despite significant sensory loss. This is the clinical paradox of DPN: the patients at highest risk of ulceration are frequently those with painless neuropathy, precisely because they have no warning symptoms to prompt care. Once protective sensation is lost, an ill-fitting shoe, a small cut, or a callus can progress to an ulcer without the person being aware of it.

Because of this, professional guidelines emphasize routine screening rather than waiting for symptoms. The American Diabetes Association recommends that all people with type 2 diabetes be assessed for DPN at diagnosis, and those with type 1 diabetes beginning five years after diagnosis, with reassessment at least annually thereafter (Pop-Busui et al., Diabetes Care, 2017; ADA Standards of Care, 2024).

How Neuropathy Is Screened

Screening for DPN does not require expensive equipment. The International Working Group on the Diabetic Foot (IWGDF) recommends a straightforward bedside examination combining a careful history with simple sensory tests (Schaper et al., Diabetes/Metabolism Research and Reviews, 2024).

The 10-gram Monofilament

The 10 g (5.07 Semmes-Weinstein) monofilament is the workhorse of foot screening. It is applied to several sites on the plantar surface of each foot; an inability to feel it indicates loss of protective sensation and a markedly increased ulcer risk. The test is inexpensive, quick, and reproducible, which is why it remains a cornerstone of the diabetic foot examination. However, clinicians should recognize its limits: a 2024 study in an Indian population found that monofilament accuracy can be reduced by factors such as occupation and thickened skin, reinforcing that it should be combined with other tests rather than used alone (Kumar et al., PLOS One, 2024).

Complementary Tests

To improve reliability, the monofilament is paired with at least one additional modality, such as vibration testing with a 128 Hz tuning fork, pinprick sensation, ankle reflexes, or temperature perception. Using more than one test increases the likelihood of catching early nerve damage, since different fibers may be affected at different stages. A 2024 multidisciplinary expert review underscored that a structured, multi-test approach improves diagnostic confidence and helps stratify which patients need the most intensive foot protection (Frontiers in Endocrinology, 2024).

Managing Diabetic Neuropathy

Glycemic Control and Risk Reduction

The foundation of DPN management is optimizing blood glucose, though its impact differs by diabetes type. In type 1 diabetes, intensive glucose control substantially reduces the risk of developing neuropathy. In type 2 diabetes, the protective effect of tight control is more modest, which suggests that other factors—such as blood pressure, lipids, and metabolic health—also contribute to nerve damage and deserve attention (Pop-Busui et al., Diabetes Care, 2017). Importantly, good glucose control can slow progression but does not reverse established nerve loss, which is why prevention and early screening are so valuable.

Treating Painful Neuropathy

For patients with painful DPN, glucose control alone rarely relieves symptoms, and medication is often needed. Guidelines recommend agents such as duloxetine, pregabalin, gabapentin, and amitriptyline as first-line options. The landmark OPTION-DM trial—the largest head-to-head crossover study of its kind—compared these drugs and their combinations and found that the major treatment pathways had broadly similar analgesic efficacy. For patients whose pain was not adequately controlled by a single drug, adding a second agent provided meaningful additional relief (Tesfaye et al., The Lancet, 2022). This gives clinicians flexibility to tailor therapy to each patient’s tolerance and coexisting conditions, rather than relying on a single “best” drug.

Foot Protection as Ongoing Care

Because painful and painless neuropathy both carry ulcer risk, sensory loss should trigger structured foot protection: regular foot inspection, appropriate footwear, prompt attention to calluses and minor injuries, and patient education about daily self-examination. For people who can no longer feel their feet, these habits substitute for the warning system that neuropathy has taken away.
