Diabetic peripheral neuropathy (DPN) is one of the most common and consequential complications of diabetes, yet it frequently develops silently over years before a person notices any symptoms. Because the loss of protective sensation it causes is the single most common contributing factor to diabetic foot ulceration — present in roughly three-quarters of people who ulcerate — identifying neuropathy early is a cornerstone of preventing wounds, infections, and amputations. This article reviews what the recent literature says about how DPN is detected, how it is managed, and where diagnosis and treatment are heading.
Why Early Detection Matters
Estimates of DPN prevalence vary widely depending on the population studied and the diagnostic criteria used, ranging from about 23% to 54% of people with diabetes. The clinical stakes are high: peripheral sensory neuropathy has been identified as a component cause in approximately 78% of diabetic foot ulcers, and the combination of neuropathy, minor repetitive trauma, and foot deformity is present in more than 60% of cases. The foot examination should also assess skin integrity, pulses in the dorsalis pedis and posterior tibial arteries, and structural deformities.
Validated questionnaires and scoring systems — including the Michigan Neuropathy Screening Instrument and the Toronto Clinical Neuropathy Score — help structure this assessment, and many societies now favor a graded framework that classifies neuropathy as possible, probable, or confirmed. It is worth understanding the limitations of individual tests. A 2024 umbrella review of screening-test accuracy found that the monofilament’s sensitivity ranged from 0.53 to 0.93 and its specificity from 0.64 to 1.00, with considerable variability in how it is applied in practice. No single bedside test is perfect, which is precisely why guidelines call for combining more than one.
Management Strategies
Management of DPN rests on two pillars: modifying the underlying disease and treating symptoms. Optimizing glycemic control is the foundation and can slow progression, particularly in type 1 diabetes, though its effect on established neuropathy in type 2 diabetes is more modest. Addressing cardiovascular risk factors and supporting foot care and regular surveillance remain essential throughout.
For the painful form of DPN, which affects a substantial minority of patients and can significantly impair sleep and quality of life, guideline-recommended first-line oral therapies include serotonin-norepinephrine reuptake inhibitors (such as duloxetine), gabapentinoids (gabapentin and pregabalin), and tricyclic antidepressants (such as amitriptyline). Reviews of current guidance note that these classes have broadly comparable effect sizes, so choice is often guided by a patient’s other conditions, potential side effects, and cost. In the United States, four agents carry specific FDA approval for painful DPN: duloxetine, pregabalin, tapentadol extended-release, and an 8% capsaicin topical system. Because long-term comparative data remain limited, treatment is typically individualized and adjusted over time.
Emerging Diagnostics and Therapies
Research is increasingly focused on detecting nerve damage earlier than traditional bedside tests allow. Small-fiber assessment techniques — including corneal confocal microscopy, which images the small nerve fibers of the cornea as a non-invasive marker of systemic neuropathy — and point-of-care devices that measure sudomotor or sensory function are being studied as tools to identify subclinical disease. Investigators are also examining blood-based and molecular biomarkers that could one day flag at-risk patients before sensation is lost. On the treatment side, interest continues in combination pharmacologic regimens and in disease-modifying rather than purely symptom-controlling approaches, although these remain areas of active investigation rather than established practice.
